Effective date: October 22, 2026. Operator: FC Salmon Creek Soccer Club, Vancouver, Washington.
This notice covers consumer health data that FC Salmon Creek may receive in connection with youth soccer programs, including from Washington families. It is separate from our general Privacy Policy, and addresses Washington’s My Health My Data Act (RCW 19.373) where applicable.
Categories of health data and reasons for collection
The club may receive information voluntarily supplied by a parent/guardian or participant about injuries and recovery, concussion-related restrictions, allergies, emergency accommodations, disability-related access needs, or fitness-to-play limitations. Such information may identify a person’s physical or mental health status. We request only what is necessary to consider a requested accommodation, respond to an incident, support safe participation, or meet a legal or league requirement. We do not ask families to provide diagnoses or medical documents through the public scholarship form.
Where health data comes from
Potential sources include parents/guardians, adult participants, information families voluntarily provide to authorized coaches or club leaders, emergency reports, and—where authorized and necessary—league or registration providers. Do not transmit detailed medical histories through public website forms, comments, or ordinary unsolicited email.
Uses of consumer health data
When such data is needed, we use it to respond to the family’s request, arrange an appropriate participation accommodation, determine any safety-related activity restrictions communicated by the family, handle club incident reporting, or comply with applicable legal duties. We do not intend to use consumer health data for targeted advertising, profiling, or the sale of information. We do not use it to publicly describe a player or evaluate athletic skill.
Categories shared and recipients
Only information reasonably needed for the specified purpose should be made available to authorized club officials or coaches involved in the requested accommodation or safety response. Depending on the circumstance, narrowly necessary information may be shared with the club’s registration or team-management provider, insurance provider, applicable youth soccer governing body, or emergency responders. Website/communications hosting and administrative IT providers may process information while providing services under club direction. The club should maintain a current record of the specific processors and affiliated entities, if any, that receive consumer health data. The club does not intend to sell consumer health data.
Consent and limitations
When Washington law applies, FCSC will limit collection and sharing to what is necessary to provide a service the consumer has requested or seek any required affirmative consent for other specified collection and separate consent for sharing. This notice is not itself a request for consent and should not be used as a substitute for a case-specific consent process. Parents should provide only the safety or accommodation information that is needed for their child’s participation.
Your Washington consumer health data rights
Under applicable Washington law, consumers may have the right to confirm whether we collect, share, or sell consumer health data; access the data and information about recipients; withdraw consent; and request deletion, subject to lawful exceptions and verification. To submit a request, contact president@salmoncreeksoccer.com with “Consumer Health Data Request” in the subject line or use our Contact page. We may seek information needed to verify identity and parental authority, but will not ask you to create a new account. We will respond within the time applicable under RCW 19.373.040; rights denials can be appealed using the same contact methods with “Health Data Appeal” in the subject.
Retention, security, and changes
Health-related records should be retained only as reasonably necessary for the requested service, legitimate safety/insurance obligations, and legally required recordkeeping, and accessed only by appropriate personnel. The club should establish and follow a documented retention, access, and deletion procedure covering coaches, platforms, and backups. We will update this notice before expanding health-data categories or purposes and obtain additional consent when required by law. For all other website data practices, see our general Privacy Policy.
